Community Health Assessment

The 2017 Community Health Assessment is a comprehensive data-driven report on the behaviors, needs and concerns of the township’s 48,000 residents. It is a follow-up to similar research conducted in 2013. More than 400 residents completed the survey (offered online and on paper; in both Spanish and English), which included questions ranging from housing and transportation to eating habits and physical activity. Four focus groups and 16 key informant interviews were also conducted to further help us identify the pressing health issues, establish priorities and goals, develop interventions, and commit resources to improve the health of the community.

Bloomfield Rapid Public Health Assessment 


Required by the Strengthening Local Public Health grant, our Local Health Outreach Coordinator completed the following Health Assessment in order to highlight the adverse health impacts of the COVID-19 pandemic and other re-emerging and emerging communicable diseases.

Community Health Improvement Plan (CHIP)


Based on the Community Health Assessment, the Department has developed a Community Health Improvement Plan (CHIP) to address some of the most challenging public health issues facing Bloomfield.


The plan describes actionable goals, objectives, and strategies for making tangible progress in several key areas including:

  • Access to screening and healthcare services;
  • Active living and healthy eating;
  • Childhood lead poisoning prevention;
  • Substance abuse prevention; and
  • Mental health programming.

Strategic Plan


The Bloomfield Department of Health & Human Services Strategic Goal is to develop and implement a proactive and efficient program plan, consistent with evidence-based best practices, to ensure the provision of vital public health services in a culturally competent manner. To that end, the Department is dedicated to upholding its Professionalism, Compassion, and Adaptability as it pursues its Strategic Goal. Our Strategic Plan outlines our mission, vision and values, as well as our priorities, goals and objectives for each division: Public Health Nursing, Health Education, Human Services, Environmental Health, and Administration. It also includes detailed SWOT (Strength, Weakness, Opportunity, Threat) analyses.

Despite many challenges, we strive to maintain and enhance our services, meet or exceed the expectations of our residents, and adapt to a changing healthcare landscape.
